Over 80% of Indians needing psychiatric care do not receive timely treatment or the right treatment. It's time mental health is treated with the same urgency as physical health, say experts.

In a country where mental health conversations are still evolving, a vast majority of Indians who need psychiatric support fail to receive timely or adequate care. Experts estimate that more than 80% of people requiring mental health treatment either never access services or seek help far too late, allowing conditions to worsen.

The Indian Psychiatric Society (IPS) has raised concern over the growing burden of anxiety, depression and other psychiatric disorders, noting that while awareness has improved in recent years, access to treatment remains severely limited. According to the IPS, nearly 80–85% of individuals living with mental health disorders remain outside the formal healthcare system. Findings from the National Mental Health Survey echo this reality, placing India among countries with the widest treatment gaps globally.

Awareness Growing, Access Lagging
Mental illnesses are highly treatable when identified early, yet many people continue to endure symptoms in silence. IPS President Dr Savita Malhotra has pointed out that stigma, poor mental health literacy and weak integration of psychiatric care into primary healthcare delay diagnosis and treatment. Emotional distress is often dismissed as everyday stress or a personal failing, preventing people from seeking professional help until symptoms become severe.

Stigma and Shortage of Specialists
Social stigma remains one of the biggest barriers, with fear of judgment from family, workplaces and society discouraging individuals from reaching out. This problem is compounded by a critical shortage of trained mental health professionals, particularly in rural and semi-urban regions where access to care is minimal.

India is estimated to have around 150 million people affected by psychiatric disorders, though experts believe the real numbers may be higher due to under-reporting. Delayed or absent treatment often leads to chronic illness, disability, family strain, loss of productivity and an increased risk of self-harm and suicide, according to mental health specialists.

Vulnerable Groups at Greater Risk
Children, adolescents and older adults are among the most affected by the treatment gap. In older populations, mental health symptoms are frequently ignored or seen as a normal part of ageing, while young people often lack supportive environments to seek help. Untreated mental illness also fuels broader social issues such as substance abuse, unemployment and family breakdown.

The Road Ahead
Government initiatives like the Tele-MANAS mental health helpline and the District Mental Health Programme have made progress, but experts stress the need for rapid expansion. Greater funding, workforce development and integration of mental health services into primary care are crucial steps forward.

Psychiatrists also emphasise the importance of breaking myths around mental illness and normalising conversations about mental health in everyday life. Closing India’s mental health treatment gap, they say, will require treating psychiatric care with the same urgency and priority as physical health—an essential step toward building a healthier and more resilient society.
