Young and at Risk: Nearly 14% of Stroke Cases in India Are Under 45

New national data from the Indian Council of Medical Research shows that nearly one in seven stroke patients in India is under 45, overturning the belief that stroke affects only older adults. Lifestyle factors like hypertension, diabetes, tobacco use and stress are major contributors to this worrying trend.

Stroke — long perceived as a disease of old age — is increasingly strikingyounger adults in India, with nationwide registry data revealing thatabout 13.8 % of stroke patients are under 45 years old. Analysis of nearly35,000 stroke cases recorded between 2020 and 2022by theIndian Council of Medical Research (ICMR)–led National Stroke Registry Programmeshows that younger adults — typically in their 30s and early 40s — are now a significant share of those affected.

Health experts link this shift to rising rates ofhypertension (present in around 74.5 % of patients), diabetes, tobacco and alcohol use, sedentary lifestyles and poor dietary habits among younger populations. These risk factors — once more common among older adults — are now prevalent in working-age groups, accelerating vascular damage that can trigger a cerebrovascular event.

Another concern is delayed treatment: justabout one in fivestroke patients reach hospital within the critical4.5-hour windowneeded for effective interventions like thrombolysis or thrombectomy. As a result, many patients facehigher rates of disability and death, especially when care arrives too late.

The findings highlight a pressing public health challenge that demands stronger focus onawareness, lifestyle modification, early screening and emergency response systems. Professionals stress regular checks for blood pressure and sugar levels, increased physical activity, tobacco cessation and stress management to reduce the risk of early-onset stroke. Prompt recognition of symptoms — often learned through theFASTmethod (Face drooping, Arm weakness, Speech difficulty, Time to seek help) — can also significantly improve outcomes.
